A list of current medications Especially anything that affects bleeding, saliva flow, or healing.

Previous dental X-rays if available Not required — we can take new ones — but recent X-rays help skip redundant imaging.

Can I request a specific doctor?
Yes. Let us know when you call and we'll match you with your preferred doctor across our three offices.
How far out are appointments available?
Routine cleanings are usually within one to two weeks. Flexible schedule? We often have earlier openings.
Can I book for my whole family?
Yes — we coordinate back-to-back appointments regularly. Let us know who's coming and we'll block time together.
Do you accept my insurance?
We work with most major PPO plans including Aetna, Cigna, MetLife, Guardian, United Healthcare, Anthem Blue Cross, and Principal.
What if I need to reschedule?
Just call us. We ask for 24 hours' notice when possible and will get you rescheduled right away.
Is there parking at each location?
Yes — all three offices have free dedicated parking. Fremont and Milpitas have surface lots; Cupertino shares a lot with neighboring suites.
