Custom Night Guards
A precisely fitted appliance that protects your teeth, jaw, and sleep from the damage caused by nighttime grinding and clenching.
Grinding is more common than you'd think
Roughly 1 in 10 adults grinds or clenches their teeth at night — and many more do it occasionally without realizing. The condition (clinically called bruxism) happens during sleep, so most patients only learn about it when a partner notices, or when we spot the telltale wear patterns at a routine cleaning.
Left alone, chronic grinding gradually wears down enamel, cracks teeth, loosens fillings and crowns, recedes gums, and overworks the muscles of the jaw. Many morning headaches and "stiff jaw" complaints trace back to overnight clenching the patient never knew about. The American Dental Association recommends a custom-fitted night guard as the first-line treatment in nearly all cases.
A night guard isn't a cure for bruxism — it's a shield. By placing a precisely fitted barrier between your upper and lower teeth, it absorbs the forces that would otherwise damage your enamel and restorations. For patients across Fremont, Cupertino, Milpitas, and the wider Bay Area, it's one of the most cost-effective preventive investments we recommend.
Symptoms patients often miss
Because bruxism happens during sleep, you're unlikely to catch yourself doing it. The clues show up elsewhere — in the morning, on the teeth, or in a partner's complaint. Any of these is worth a conversation at your next visit.
If you have several of these together, the case for a night guard is strong even without imaging — we can often see the evidence in your wear pattern within seconds.
Custom vs. over-the-counter
Drugstores and online sellers offer night guards starting around $20. They look like a cheap, reasonable option — but they're a very different product than what we make in the office. Here's the honest comparison.
| Over-the-counter | Custom dental guard | |
|---|---|---|
| Fit | "Boil-and-bite" — approximate | Precisely molded to your teeth |
| Comfort | Often bulky, hard to wear nightly | Slim, comfortable enough for nightly use |
| Material | One-size soft plastic | Hard, soft, or layered — chosen for your case |
| Protection | Partial — can break under heavy grinding | Engineered for sustained grinding forces |
| Lifespan | 6 months to 1 year | 3 to 5 years typical |
| Cost | Low upfront, replaced often | Higher upfront, lower over time |
| Side effects | Can shift bite over time if ill-fitted | Designed to preserve bite alignment |
How a custom guard is made
Getting a custom night guard is straightforward — typically two short visits about 2 weeks apart. The actual chair time is minimal; most of the wait is your guard being precisely crafted at a dental lab.
- Evaluate your bite Your doctor examines your teeth, jaw, and bite to confirm grinding patterns and choose the right guard material for your case.
- Take a digital scan A 3D scan captures your bite — no goopy impressions. Sent to the lab for fabrication.
- Lab crafts your guard Typically 1–2 weeks. The guard is precisely milled or molded to your exact bite anatomy.
- Fitting & adjustment You return for a fitting. Your doctor checks the fit, adjusts as needed, and you take it home — ready to use that night.
Making your guard last
A well-cared-for custom night guard lasts 3 to 5 years, sometimes longer. A few small habits keep it in good shape for the full lifespan.
Daily routine
- Rinse with cool water after removing it each morning.
- Brush gently with a soft toothbrush — no toothpaste (it's too abrasive).
- Air dry on a clean surface before storing.
- Store in its case, never in a napkin or pocket where it gets lost or crushed.
Weekly & long-term
- Soak weekly in denture cleaner or a vinegar-water solution to prevent buildup.
- Avoid hot water — can warp the material out of shape.
- Bring it to cleanings. We check the fit at every routine visit.
- Replace when worn through, not before. Many last well beyond 3 years.
Things patients often ask
How do I know if I grind my teeth at night?
Most people don't catch themselves, but common signs include morning jaw soreness, tension headaches on waking, worn or flattened teeth, unexplained chips or cracks, and a sleep partner hearing the grinding. At your routine cleaning, your doctor can spot the wear patterns and confirm.
How much does a custom night guard cost?
Custom night guards typically range from $300 to $600 depending on the material and complexity. Many PPO dental plans cover at least part of the cost when grinding is documented. We provide a full estimate and verify your benefits before scheduling.
Will it feel uncomfortable to sleep with?
The first few nights take adjustment — most patients describe it as feeling unusual, not uncomfortable. By the end of the first week, the vast majority of patients sleep through the night without noticing. Custom-fit guards are dramatically more comfortable than over-the-counter alternatives.
How long does a night guard last?
A custom night guard typically lasts 3 to 5 years with proper care. Heavy grinders may need replacement sooner; lighter grinders may get more than 5 years. We check the fit at every routine cleaning.
Can a night guard fix my TMJ pain?
Often, yes — at least the muscle-related component. By distributing bite forces and preventing clenching, a night guard reduces the muscle strain that drives most TMJ pain. More complex TMJ disorders may need additional treatment, which we'd discuss after the guard has had a chance to help.
Does my child need a night guard if they grind?
Usually not. Childhood grinding is common and most kids outgrow it as their permanent teeth come in and their bite settles. We monitor for unusual wear patterns at routine visits and recommend a guard only when there's clear damage to permanent teeth. For most children, watchful waiting is the right approach.
